NECESSARY DOCUMENTS FOR ALL STUDENT-ATHLETES

FinalForms:  All student-athletes must be registered in FinalForms.  Click on this link to view the document that will walk you through the process of creating an account in FinalForms.  When creating your account, please use an email address and password that you will remember.  This account will be a part of your student-athletes time at White Pine and at Heritage High School.  Please use your student's school email account when prompted for a “student email”.  Once you have created an account, use this link to return to your account.

 

MHSAA Physical:  All student-athletes must have a current physical uploaded into FinalForms each year.  The physical must have occurred on April 15 or later of each year. For example, if the school year is 2025-26, the physical must be on April 15, 2025 or later.  Please download the MHSAA physical Form and make sure that everything is completed; student's name, doctor's name, doctor's phone number, actual date of exam and student's birthdate. The medical professional will fill out their information and sign and date this form.
